Its a driver issue. I doubt its your card. This was happening to to me as well. Nvidia just release new drivers. Download those and it should fix the problem. The new drivers finally fixed the problem for me. If the new drivers do not fix the problem, I'll tell you what drivers may fix the problem.
What model is your video card.?

This was a common problem I remember reading about. Give this a shot. Use the latest beta drivers from Nvidia's website.
If you have a 7 series use the below.
ForceWare Release 169 BETA Version: 169.28 Release Date: January 10, 2008

Here's an example of a guy that used the latest drivers, but not the beta ones. His results were similar to your's until he installed the beta ones.

Thanks guys for the info.
I don't think it overheating, this only happens with COD4.
I have a Nvidia GeForce 7900GTX card with driver v6.14.11.6218
Forceware v162.18
Well I updated my drivers, and it is looking and acting better, Thanks Rajun!

Yeah same here. I liked the 7900 gtx, but I'm sick of the driver problems.
